Disorders of the intestinal flora are associated with various diseases, including colitis and colon cancer. A variety of immune cells in the body can regulate the intestinal flora by secreting exosomal vesicles, which carry multiple micrornas. Then, under natural conditions, the number of exosomal vesicles secreted by these immune cells is very small and insufficient to regulate the imbalanced intestinal flora. In this paper, an implanted hydrogel device was utilized to encapsulate autologous macrophages. This hydrogel device can promote macrophages to produce twenty times the number of exosomal vesicles under mechanical forces and photoelectrical signals, thereby actively regulating the intestinal flora. Therefore, macrophages in hydrogels act like factories for exosome production, constantly regulating the microbiota by releasing exosomal vesicles.
